Where can I get an intake form?
- Before the first session all clients will fill out an Intake Form.
This form is designed to let the counselor know a little bit
about you. It is also so you can know a little more about Bridging
Counseling. The form will be talked about during the first session,
and you are welcome to ask any questions. To view an intake form, including a sliding scale fee, please click on the link above.
- Counseling
is intended to be affordable for the general public. A sliding scale
is used to the benefit of many clients. Therefore, fees may vary from
$50 to $90 depending on one's gross income. Counselors may be able to
accept students and other persons that cannot afford counseling for a
minimal amount that will be discussed in session. Ask your counselor about discounts for Hillcrest Chapel attenders and students. Bridging also has a counseling intern currently in graduate school that will see clients at a discounted rate.
- No. Insurance payments will be more widely accepted in the future. Right now, payments can be made by check or cash. Checks can be made out to Hillcrest Community Services.
- Yes, Amy McMurtrey works primarily with adolescents and younger children. She was trained at Seattle Pacific University in play therapy and family counseling.
